Clinical Records
We are required by professional and funding standards to maintain a record of the work we do and how well we are doing in achieving your specified goals. Our files are open for you to read. They are stored in a secure location.

Confidentiality
Information that you share with us is strictly confidential. Legally, information and reports in your file cannot be shared with anyone without your signed consent. You may see your file or consent to others receiving copies of our reports during the time you are receiving service at Integra and for approximately 10 years after service ends.

Child Abuse & the Law
There is an important exception to the confidentiality of what you tell us. We are required by law to report to the Children's Aid Society if there is any reason to suspect that your child or teen has been abused.

What you can do if you are unhappy with the service you receive
If you are unhappy with the service you receive at Integra, we strongly encourage you to talk with your Integra worker. If you are not satisfied, or you do not feel comfortable about approaching your worker, you may contact the Clinical Director, the Executive Director, or a member of the Board of Directors. If the matter is still not resolved to your satisfaction, you may contact the Ministry of Community, Family and Children's Services.

Emergency
Although Integra does not have a crisis service, you may contact your worker or a supervisor during the hours that Integra is open. If you have an emergency after hours, we suggest that you contact: a hospital emergency room, a Children's Aid Society, Youthdale Crisis Service, or a telephone help line.

Referrals
Referrals to Integra can be made by parents, youth, legal guardians, or professionals. For more information, or to speak to the Intake Worker, please telephone (416) 486-8055.

Please Note: We do not accept referrals online due to confidentiality concerns.
